By purchasing or using any product from USADoctorList.com, you agree to be bound by these Terms of Service. If you do not agree, do not make a purchase.

2. What We Sell

USADoctorList.com sells digital CSV database files containing publicly available US medical professional records sourced from the NPPES (National Plan and Provider Enumeration System), a database maintained by the Centers for Medicare & Medicaid Services (CMS).

3. License & Permitted Use

Each purchase grants you a single-user, non-exclusive license to use the data for your own internal business purposes, including:

  • Marketing outreach and lead generation
  • Research and analysis
  • CRM imports for your own business

You may not:

  • Resell, sublicense, or redistribute the data files to third parties
  • Share purchased files publicly or post them online
  • Use the data in a way that violates applicable law, including CAN-SPAM, TCPA, or HIPAA where applicable

4. Download Links

After a successful payment, download links are delivered by email and displayed on the confirmation page. Links are valid for 5 days from the time of purchase and allow a maximum of 5 downloads per file. We strongly recommend downloading your files immediately. Expired links cannot be automatically reissued — contact us if you have an issue.

5. Data Accuracy

Our data is sourced from the NPPES database (March 2026 release). While we strive to provide accurate and up-to-date information, we make no guarantees regarding the completeness or accuracy of individual records. Medical professionals may have changed addresses, retired, or had their licenses updated after the NPPES data was compiled. Use the data at your own discretion.

6. Payments

All payments are processed by PayPal. Prices are listed in US Dollars (USD). By completing a purchase, you authorize PayPal to charge your selected payment method for the total shown at checkout.

Please see our Refund Policy for full details. In summary, we offer a satisfaction guarantee — if the data does not meet your needs, contact us within 7 days of purchase.

8. Limitation of Liability

USADoctorList.com is not liable for any indirect, incidental, or consequential damages arising from your use of our data. Our maximum liability is limited to the amount you paid for the purchase in question.

9. Changes to Terms

We reserve the right to update these Terms at any time. Continued use of the site after changes constitutes acceptance of the updated Terms.
